Patients: To request a copy of your emergency medical report, please provide photo identification and complete the above Authorization Form.
Attorneys and other Agencies: Attorneys representing a patient should provide the following when requesting a medical record:
1. A letter of request on your letterhead that includes the patient's full name, incident date, incident time, and incident location.
2. An authorization to release the record that:
- Is in writing, dated, and signed by the patient
- Identifies Eastside Fire & Rescue as the health care provider
- Identifies to whom the record is to be provided
- States the time period the authorization will be valid, which may not exceed one year. NOTE: Authorizations missing any of the required elements will not be accepted.
Attorney Representing a Duly Appointed Personal Representative: Attorneys representing a duly appointed personal representative should provide the following when requesting a medical record:
1. A letter of request on your letterhead including the patient's name, incident date, incident time, and incident location
2. A copy of the document by which the person is the duly appointed personal representative
3. A copy of the Death Certificate
4. An authorization releasing the record that:
- Is in writing, dated and signed by the personal representative
- Identifies Eastside Fire & Rescue as the health care provider
- Identifies the patient
- Identifies to whom the record is to be provided
- States the time period the authorization is valid, which may not exceed one year. NOTE: Authorizations missing any of the required elements will not be accepted.

Eastside Fire & Rescue does not maintain records for the following:
ENVIRONMENTAL Records
Reports for underground and above ground storage tanks are maintained through City and County Permitting offices. Please navigate within the appropriate website to obtain the city / county Public Record Request Forms.
HAZMAT Records
Please reference the Department of Ecology website to obtain specific property research associated with Hazardous Material storage.
911 AUDIO or WRITTEN Records
For records and information documenting call times, as well as resources dispatched to fire/aid calls within Eastside Fire & Rescue’s boundaries, submit your requests through NORCOM.
FIRE INVESTIGATION Reports
These records are primarily documented findings regarding the cause of a fire. **Please Note: Each fire is unique and may not require a fire investigator to complete a report. Contact the King County's Fire Investigation Unit for these requests.
KING COUNTY SHERIFF’S OFFICE Records
These records pertain to any documentation made in the line of duty by a King County Sheriff’s Officer that substantiates generating a case number for their response. Contact King County Sheriff's Office for these requests.
ISSAQUAH, SNOQUALMIE, NORTH BEND POLICE DEPT Records
These records pertain to any documentation made in the line of duty by a Police Officer that substantiates generating a case number for their response. Contact the appropriate city's police department for these requests.
WASHINGTON STATE PATROL Records
These records pertain to any documentation made in the line of duty by a WSP Officer that substantiates generating a case number for their response. Contact the Washington State Patrol for these requests.
