Environmental Record Requests
Reports for underground and above ground storage tanks are maintained through City and County Permitting offices. Below is a list of these offices that are within Eastside Fire & Rescue’s jurisdiction, please navigate within these websites to obtain their Public Record Request Forms.

You will need to be prepared to prove identity for HIPAA-Privacy Rules compliance. If mailing or emailing the authorization form, please include a copy of your current valid driver license. For records that include documents associated with transportation billing for a recent incident, requests may take 10-15 business days.
